Looks like Ms.Nicole/Dr.Ansari assigned some referrals. Did you get the referral Copy from the office? Please let us know if there are any issues creating an appointment.

Looks like Ms.Nicole/Dr. Ansari send some medications to your pharmacy (say the pharmacy from the chart). Were you able to pick up the medication already? Please let us know if the pharmacy didn’t receive the order.
