Patient Checkin

The purpose of the Patient Check-In process is to make sure all fields are being populated with the correct data. 
- The front desk will be responsible for checking patients in 
- Today's appointments with all relevant data can be access from Today Appointments Report
- Front office will be responsible for verifying all data of patients seen. Those reports can be accessed:

1. Data to be Verified & Updated

Amazing Charts

1 - First Name & Last Name
2 - Date of Birth
3 - Patient Address (Street, city, state & zip)
4 - Pharmacy (preferred)
5 - Language Preference
6 - Race
7 - Ethnicity
8 - Phone
9 - Email
10 - Contact Preference
11 - Ins Plan Name
12 - Ins ID
13 - VFC Reason
14 - Sexual Orientation
15 - Gender Identity

Veins

1 - First Name & Last Name
2 - Date of Birth
3 - Phone & Email
4 - External ID
5 - Visit Type/Reason
6 - Tasks To Do
7 - Appointment Time
8 - Group Assignment

- All of these fields must be verified for all patients

2. Patient Checked-In on Vein's

Map Patients

Click on "Map Patient" icon 
 

- If current patient - Find and map to patient profile
- If new patient - New Patient is identified as profile not found
Always search for patient if available!

Data can be updated from the last column



Cross demo against Amazing Charts

Open Veins Patient Profile


Cross verify and update information in Amazing Charts 
1 - First Name & Last Name
2 - Date of Birth
3 - Phone & Email
Update as necessary
- Phone and Email should be most current in both!


Make sure Amazing Charts Patient ID is in External ID
4 - External ID


Verify Visit Details

Open Visit Details


Ensure the correct information is filled out in visit details!
5 - Visit Type/Reason
6 - Tasks To Do
7 - Appointment Time

Check for any tasks due


Lastly all patients in a family must be grouped together. 
8 - Group Assignment



3. Verify Insurance & VFC Status

Check Popup Note

If patient verification was completed notes will be left in the popup note



IMPORTANT:
ALL insurance must end with "- Medicaid" or - " - Commercial"
If not. UPDATE


Verify VFC

If 
- Commercial Insurance
- Older than 18
VFC status should be "Not VFC Eligible"


If 
- Medicaid Insurance
- Under 18 years Old
VFC status should be "VFC eligible - Medicaid/Medicaid Managed Care"


If 
- No Insurance
- Under 18 years Old
VFC status should be "VFC eligible - Uninsured"


4. Verify Patient Demo in Amazing Charts

General Info

1 - First Name & Last Name
2 - Date of Birth
3 - Patient Address (Street, city, state & zip)
4 - Pharmacy (preferred)
ONLY 1 PREFERRED PHARMACY


5 - Language Preference
6 - Race
7 - Ethnicity


8 - Phone 
9 - Email
10 - Contact Preference


14 - Sexual Orientation (Check new patient packet)
15 - Gender Identity (Check new patient packet)
Both field MUST BE POPULATED


5. Check-In Complete

Move to "Ready to be Roomed" or "Tele-Visit"
